Birth Tub Rental Agreement

 

The undersigned agrees:

1. It is recommended that births be attended by a birth professional.

2. Rental pool is limited to indoor use.

3. To take full responsibility for the manner in which the pool is used.

4. To pay for the replacement of the pool or parts/accessories if they are damaged or rendered unusable while in my possession.

5. The pool kit will be returned undamaged and thoroughly cleaned following use, as outlined in the instructions included with the pool kit.

6. To pay for any shipping charges both ways, if required or chosen.

7. To release and hold harmless Kerri Smith, AquaDoula, or any agent thereof, from any responsibility/liability for maternal complications, infant mortality or morbidity, or injury to any person connected with the use of the pool that may occur before, during, or after labour/birth.

8. To pay an additional $100 cleaning fee if pool is returned in any condition other than sanitary.

9. Pool will not be shipped until price of rental, taxes, and shipping is paid.

10. A non-refundable $75 deposit, applied against the total balance owing, will hold your place on the calendar.

The rental price is $250 (Canadian), plus shipping if required.
